ASSUMPTIONS:

1. If services are required to be performed on-site, travel costs, meals/per-diem, and incidentals are billed in addition on actual.

2. If services are required to be performed on-site, services must be scheduled at least two weeks in advance

3. Scheduling of services is subject to availability of Unifyed resources

4. 100% premium to the rate is added for weekends and holidays and is subject to availability of Unifyed resources
